- Maintain accurate and timely documentation in compliance with regulations.
- Certified Social Worker - Required
- 2 years of MSW experience in Home Health or Hospice setting - Required
- Experience in Hospice or palliative care - Required
- Strong interpersonal and counseling skills.
- Valid Drive License and reliable transportation.
