Role Description This is a full-time, on-site Physical Therapist role based in Lafayette, CO. The Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver therapeutic interventions to improve mobility, reduce pain, and restore function. Daily responsibilities include performing comprehensive assessments, documenting clinical findings and progress notes, educating patients and families on home exercise programs, and coordinating care with referring providers and other team members. The role also involves participating in case discussions, maintaining current knowledge of best practices, and contributing to a positive, inclusive clinic culture.
Qualifications
- Demonstrated knowledge of Medicine and Physical Therapy appropriate to outpatient orthopedic and rehabilitative care.
- Strong Rehabilitation and Patient Care skills, including treatment planning, therapeutic exercise, and manual therapy.
- Ability to support patients through Home Care guidance, including clear instruction in home exercise and self-management strategies.
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program; eligibility for Colorado Physical Therapist licensure.
