What You’ll Do
- Coordinate Essential Care: Schedule, track, and manage medical, dental, vision, and wellness appointment to ensure continuity of care.
- Keep information Organized & Compliant: Maintain accurate records, monitor documentation, and ensure information stays current, secure, and aligned with requirements.
- Contribute to Direct Services: Provide direct services three days per week to stay connected to the consumer experience and understand the impact of services firsthand.
- Communicate & Advocate: Work closely with individuals, families, providers, and internal teams to ensure appointments and care plans run smoothly.
- Support Community Inclusion: Help connect people to resources, events, and opportunities that promote wellbeing and participation.

- Education & Experience:
- Relevant degree (preferred). High school diploma or equivalent required.
- Experience in healthcare coordination, scheduling, records management, or social services is a plus.
- Skills:
- Strong organization and attention to detail. Clear, professional communication.
- Able to work independently and manage multiple priorities.
- Passion for Inclusion: A genuine desire to support individuals with disabilities.
- Requirements: Valid driver’s license and reliable transportation.
