What are the responsibilities and job description for the Family Medicine Nurse Practitioner position at Abigail Family Medicine?
Abigail Family Medicine is a busy family practice located in Newark, DE that has been owned and operated by a solo-physician for the past 20 years, Christine E.K. Horah, D.O. Dr. Horah is looking to add a Nurse Practitioner to her practice in order to grow Abigail Family Medicine and expand medical services offered to the surrounding community.
In this position you will demonstrate knowledge and skills necessary to provide care to adolescent, adult and geriatric patients in the primary care setting. Duties include but are not limited to:
- Provide medical care and examinations of patients for acute and chronic conditions.
- Perform annual wellness visits, manage transitional care patients and assist patients with advanced care planning.
- Triage walk-in patients and ensure appropriate disposition.
- Answer and respond to telephone messages.
- Function as a liaison between patients, physicians and staff to provide accurate communication.
- Support the work of LPN and/or MA.
Requirements:
- Licensed to practice in the State of Delaware.
- Current federal and state controlled substance certificate.
- New graduates will be considered.
