Role Description The Hospice Liaison is a full-time, on-site role based in Oklahoma City, OK, responsible for building and maintaining relationships with healthcare providers, facilities, and community partners to promote Abiding Hospice services. Day-to-day responsibilities include meeting with referral sources, explaining hospice and palliative care options, and coordinating smooth transitions for patients and families into hospice care. The role involves conducting assessments, collaborating with clinical teams to ensure appropriate care plans, and educating families about available services and resources. The Hospice Liaison also documents outreach activities, tracks referrals, and participates in community events and presentations to expand awareness of the organization.
Qualifications
- Strong knowledge and experience in Hospice Care and End-of-Life Care to guide patients, families, and referral sources effectively.
- Understanding of Palliative Care principles to collaborate with clinical teams on appropriate care options and support.
- Proficiency in Sales and relationship-building to develop and maintain referral networks and community partnerships.
- Familiarity with Home Care services to coordinate care transitions and align support across care settings.
- Relevant clinical or healthcare background (e.g., RN, LPN, social work, or similar experience) preferred.
- Excellent communication, presentation, and interpersonal skills, with the ability to explain complex care options clearly and compassionately.
- Ability to work collaboratively with interdisciplinary teams and manage time effectively in a fast-paced environment.
- Valid driver’s license and reliable transportation for on-site visits within the community.
