We are a compassionate and patient-centered home care agency proudly serving the Stoneham, MA area. Our mission is to provide high-quality, personalized care in the comfort of our clients' homes. We are currently seeking experienced and dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our per diem team.
Position Summary:
As a per diem home care nurse, you will be responsible for overseeing our clients care in their homes. This is an excellent opportunity for nurses seeking flexibility, autonomy, and meaningful one-on-one patient care interaction.
Responsibilities:
- Perform initial and ongoing nursing assessments to determine client needs.
- Develop, implement, and revise personalized care plans
- Provide education and health coaching to clients and caregivers on disease management, medications, and home safety
- Coordinate services and follow-ups with ASAP's ( Aging Service Access Point) as well as family members
- Supervise and evaluate home health aides and ensure plan-of-care compliance
- Maintain accurate and timely clinical documentation in accordance with regulatory and agency standards
- Participate in quality assurance activities and support compliance with state and home health regulations
Requirements:

Current MA RN or LPN license in good standing

Minimum of 1 year of clinical nursing experience (home care experience preferred)

CPR certification

Reliable transportation and valid driver’s license

Strong communication, organizational, and critical thinking skills
