At Aberdeen, we provide high-quality, compassionate care to individuals in the comfort of their own homes.
A valued member of our case management team is retiring, and we're looking for the right person to step in. This is a primarily office-based role with regular travel to client homes on the North Shore and occasional visits to hospitals, facilities, and physician offices.
We're looking for someone who gets what makes home care different. This role requires clinical strength, but it also takes heart, patience, and the kind of steady presence that makes everyone around you feel like things are going to be okay.
Key Responsibilities
- Intake assessments, case management, skilled nursing visits, care plans, and caregiver supervision.
- Serve as the primary clinical contact for families, keeping communication warm, timely, and transparent.
- Provide direct patient care as needed.
- Participate in a rotating clinical on-call schedule.

Requirements
- MA RN license, current and in good standing.
- Minimum 5 years of clinical nursing experience.
- Management experience a bonus!
- Working knowledge of MA privacy standards and HIPAA.
- Strong communication, leadership, and clinical judgment.
- Comfort with technology and willingness to learn agency software.
- Fluent in English. Valid MA driver's license and reliable transportation.
