⭐ Who we’re hiring
We’re seeking a Home Health PT to serve patients across Rockford and surrounding communities.
🧠 What you’ll do
- 🏠 Conduct comprehensive in‑home PT evaluations and build individualized therapy plans
- 💪 Deliver skilled therapy: therapeutic exercise, balance & gait training, manual techniques, patient/caregiver instruction
- 🔄 Monitor progress and adjust plans; collaborate with physicians and the interdisciplinary care team
- 👨👩👦 Educate patients and families on HEPs, safety, equipment, and independence at home
- 📝 Complete required EMR documentation (evaluations, visits, reassessments) and support safe discharge planning
🎯 What we’re looking for
- ✔️ Active Illinois PT license (DPT or equivalent)
- ✔️ Home Health or comparable clinical experience preferred
- ✔️ Strong assessment, communication, and documentation skills
- ✔️ Comfort working independently in varied home settings; collaborative with RN/OT/SLP/HHA teams
📅 Schedule & training
- 🔟 Minimum 5 visits per week
- 🗓️ Flexible scheduling you design
- 🎓 Paid, tailored training (~2 weeks) in Arlington Heights before you take your own caseload
