Welcome Homecare is seeking Full-Time and Part-Time Home Health RNs to assist our patients in Charlottesville and Albemarle County, VA.
- Assess and chart observations of the patient's condition at each visit.
- Complete evaluation tasks, including reviewing medication and vital signs.
- Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient's care plan.
Requirements:
- Current RN license
- At least one (1) year of recent home health experience
- Able to work in the community, drive and meet patients in their home
- Must be authorized to work in the US
