Position Summary:
We are seeking dedicated and experienced Geriatric Nursing
Assistants (GNAs) or Certified Nursing Assistants (CNAs) to join our
home health team. The ideal candidate will have 1-3 years of
experience in nursing homes or long-term care facilities and a passion
for delivering high-quality care in a home setting.
Key Responsibilities:
- Provide personal care services, including bathing, grooming,
and dressing.
- Assist with mobility, transfers, and
ambulation, including the use of assistive devices.
- Monitor and record vital signs, such as blood pressure,
temperature, and pulse.
- Offer companionship and
emotional support to clients.
- Administer medication
reminders as directed by the care plan.
- Perform
light housekeeping, meal preparation, and errands.
- Ensure patient safety and report any changes in the client’s
condition to the supervisor or RN.
- Document care
provided in compliance with agency standards.
Qualifications:
- Current GNA or CNA certification in [State Name].
