Registered Nurse (RN)

Position Summary

The Registered Nurse (RN) for the Adult Day Health (ADH) Program is responsible for overseeing and coordinating participants’ clinical and healthcare needs while ensuring compliance with all applicable Massachusetts Department of Public Health (DPH), MassHealth, and organizational policies and procedures. The RN promotes participant health, safety, independence, and quality of life through assessments, care coordination, medication oversight, participant education, and interdisciplinary collaboration.

The RN serves as an essential member of the ADH interdisciplinary team and assists in maintaining a safe, professional, and participant-centered environment.

Essential Duties and ResponsibilitiesClinical Responsibilities

• Conduct nursing assessments and ongoing participant health evaluations.
• Monitor participant health status and report changes in condition promptly.
• Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
• Administer medications and treatments as ordered by physicians and according to agency policy.
• Review physician orders, hospital discharge summaries, and medication changes.
• Monitor vital signs, blood glucose levels, and other clinical indicators as needed.
• Respond appropriately to participant medical emergencies and incidents.
• Ensure infection control and universal precautions are consistently followed.

Care Coordination

• Coordinate participant healthcare services with physicians, hospitals, specialists, pharmacies, caregivers, and case managers.
• Communicate effectively with families and caregivers regarding participant health concerns and updates.
• Participate in interdisciplinary team meetings and care conferences.
• Assist with participant admissions, re-admissions, and discharges.

Documentation & Compliance

• Maintain accurate, timely, and compliant clinical documentation.
• Ensure participant records meet MassHealth, DPH, CARF, and organizational standards.
• Complete incident reports and follow-up documentation when required.
• Assist with audits, surveys, quality assurance activities, and regulatory inspections.

Participant Support & Education

• Educate participants and caregivers on disease management, medications, nutrition, and safety.
• Promote participant dignity, independence, and emotional well-being.
• Support participant engagement in ADH services and activities.

Job Types: Full-time, Part-time, Per diem

Pay: $42.00 - $45.00 per hour
