You will partner with Nurse Practitioners and Physician Assistants to manage post-acute and long-term care patients, provide clinical leadership within assigned facilities, and ensure high-quality, patient-centered care.
Key details
- Part-time: typically 1-2 days per facility per week
- No fixed daily start/end times; no overnight shifts; no call
- Typical volume: 18-25 visits per day
- Company-provided iPad with LTE for documentation

Requirements
