We’re looking for a Family Medicine Nurse Practitioner to join our care team and deliver practical, evidence-based primary care to adults and families in Tacoma. - Collaborate with patients and the care team—including an onsite Whole Health Coach—to build biopsychosocial care plans that respect each patient’s background and goals.
- Provide comprehensive primary care under the medical home model: take thorough histories, perform appropriate physical exams, order and interpret diagnostic testing, prescribe medications, and use onsite lab and pharmacy resources for convenience.
- Identify population needs and work with teammates to increase participation in annual whole health evaluations, health coaching, and other wellness activities.
- Serve as a clinical leader by participating in team meetings, communicating proactively, prioritizing initiatives, contributing to process improvement, and supporting change management efforts.
- Rotate with other regional providers to provide after-hours coverage as needed.
