Position Summary
The Designated Coordinator/Designated Manager (DC/DM) is responsible for the oversight, coordination, and compliance of services delivered under Minnesota 245D HCBS standards. This role ensures all services are person-centered, compliant with DHS requirements, and aligned with each individual’s CSSP and CSSP Addendum.
This position combines both Designated Coordinator (service coordination) and Designated Manager (program oversight and compliance) responsibilities.
Key ResponsibilitiesService Oversight & Coordination
- Oversee implementation of CSSP and CSSP Addendum for all assigned individuals
- Ensure services align with authorized units, outcomes, and person-centered preferences
- Coordinate care with case managers, guardians, and interdisciplinary teams
Regulatory Compliance (245D Core Requirement)
- Ensure full compliance with Minnesota 245D licensing standards
- Maintain and review:
- CSSP Addendums
- Progress Review Reports
- Incident and emergency reports
- Individual Abuse Prevention Plans (IAPP)
- Ensure compliance with required timelines:
- 45-day planning meeting
- 60-day addendum updates
- Annual reviews
Staff Supervision
- Supervise and support direct support professionals (DSPs)
- Ensure staff complete all required training (245D, Vulnerable Adult Act, Positive Support Rule)
- Provide ongoing coaching and performance oversight
Health, Safety & Risk Management
- Monitor safety of all persons served
- Review and respond to incidents and reportable events
- Ensure implementation of abuse prevention and risk mitigation strategies
Documentation & Quality Assurance
- Audit staff documentation for accuracy and compliance
- Ensure timely completion of all required reports
- Prepare for DHS licensing visits and quality assurance reviews
Positive Supports & Person-Centered Practices
- Ensure services follow person-centered planning principles
- Oversee implementation of positive support strategies
- Promote independence, dignity, and community inclusion
Minimum Qualifications (Must Meet DHS Standards)Education & Experience
Candidate must meet one of the following:
- Bachelor’s degree in human services (or related field) 1 year experience
- Associate degree 2 years experience
- Diploma/certificate 3 years experience
- No degree 4 years full-time relevant experience
Required Competencies
- Strong knowledge of MN 245D regulations
- Experience working with individuals with disabilities
- Ability to supervise staff and manage compliance systems
- Strong organizational and documentation skills
Preferred Qualifications
- Prior experience as a DC or DM in a 245D program
- Experience with ICS, IHS, Homemaking, or Night Supervision
- Familiarity with MN-ITS and DHS systems
- Knowledge of Positive Support Rule (MN Rule 9544)
Work Environment
- Combination of remote administrative work and in-person service oversight
- Travel required for client visits, staff supervision, and compliance monitoring

Pay: From $4,000.00 per week
