Corewell Health in Grand Rapids, Michigan is seeking a compassionate, mission-driven Family Medicine Physician to join our innovative Intellectual & Developmental Disabilities (IDD) Integrated Practice.
This is a unique opportunity to help build and shape a specialized, interdisciplinary care model designed to meet the complex needs of patients age 16 years and up with moderate to severe IDD. The primary care physician in this practice will play a key leadership role in advancing care delivery, improving outcomes, and ensuring a sustainable, patient-centered model.

This is a unique opportunity to help build and shape a specialized, interdisciplinary care model designed to meet the complex needs of patients age 16 years and up with moderate to severe IDD. The primary care physician in this practice will play a key leadership role in advancing care delivery, improving outcomes, and ensuring a sustainable, patient-centered model.
Position Highlights
- Innovative care model that focuses on the IDD population providing whole-person care
- Balanced role with leadership responsibilities that will be approximately 40% traditional primary care, 40% primary care for the IDD population, and 20% administrative time
- Extended visit times for the IDD population with longer, more meaningful patient and family interactions
- Multidisciplinary clinic with a behavioral analyst (BCBA), social work, nurse navigator and psychiatry support
- Purpose-build clinic with provider, patient and family needs at the center
- Provide comprehensive primary care services for patients age 16 with moderate to severe IDD
- Collaborate with interdisciplinary team members to coordinate complex care needs
- Support development and growth of the practice model, including care pathways and best practices
- Build strong relationships with internal and external stakeholders
- Participate in education and advocacy efforts for IDD care across the system and community
- Contribute to program innovation and quality improvement initiatives
- MD or DO, Board Certified or Board Eligible in Family Medicine
- Interest in, or passion for caring for individuals with IDD
- Fellowship training in IDD preferred, but not required
- Open to new graduates/residents with a strong interest in this population
