Job Overview
The Case Manager coordinates and manages care for clients in outpatient behavioral health programs, ensuring that each client receives comprehensive, timely, and effective services. This includes assessing client needs, developing individualized treatment plans, facilitating linkages to community resources, monitoring progress, and ensuring continuity of care across service levels.
Essential Functions
- Conduct comprehensive psychosocial assessments and intake interviews to determine client strengths, needs, risks, and barriers.
- Develop individualized service/treatment plans in collaboration with clients and clinical team members, setting measurable goals and interventions.
- Coordinate and follow up on referrals to behavioral health clinicians, primary care, substance use treatment, housing, employment, and community resources.
- Monitor client progress toward goals, document outcomes, and update plans as needed.
- Advocate on behalf of clients to remove barriers to service access and ensure continuity of care.
- Collaborate with interdisciplinary team members to ensure integrated, trauma-informed care.
- Maintain accurate and timely documentation in accordance with agency policies and regulatory requirements.
- Provide crisis intervention and supportive services as needed, coordinating with emergency or higher-level care resources.

- Maintain regular and reliable attendance.

Education / Experience / Skill Requirements
- QMHP with Bachelor’s degree in Social Work, Psychology, Counseling, or a related human services field (Master’s preferred).
- Minimum of 2 years of experience in behavioral health case management or related service delivery.
- Knowledge of behavioral health diagnoses, treatment modalities, community resources, and service systems.
- Strong communication, organizational, and problem-solving skills.
- Ability to manage a caseload and work both independently and collaboratively.
- Proficiency in documentation and electronic health record (EHR) systems.
- Cultural competence and ability to engage clients in a trauma-informed manner.
Licenses / Designations / Certifications
- Valid driver’s license and reliable transportation required.
- Certification in Case Management (CCM) preferred.
- CPR and de-escalation/restraint certification required (training provided upon hire).
- First Aid certification may be required based on state or facility requirements.
Job Type: Full-time
Pay: $40,000.00 - $50,000.00 per year
