Hospice NP

NP Job Description

We are seeking a compassionate and skilled Hospice Nurse Practitioner (NP) to join

our dedicated team at Aachele Home Care and Hospice. As a Hospice Nurse

Practitioner, you will play a pivotal role in providing high-quality care for patients with

life-limiting illnesses, offering comfort and support to patients and their families.

You will collaborate with physicians, nurses, and other healthcare providers to develop

and implement individualized care plans

Responsibilities:

 Perform comprehensive patient assessments, medical history reviews, and

symptom management evaluations.

 Develop, implement, and revise patient care plans with the interdisciplinary team.

 Communicate effectively with Medical Director, interdisciplinary team, patients,

and families, addressing their concerns and ensuring their needs are met.

 Provide guidance and leadership to hospice nurses, social workers, chaplains,

and other team members to ensure high-quality patient care.

 Monitor patient progress, manage pain and other symptoms, and adjust care

plans as needed.

 Conduct home visits to assess patient conditions, offer emotional support, and

educate families on the hospice process and care requirements.

 Ensure documentation is up to date and compliant with regulatory standards.

Qualifications:

 Master of Science in Nursing (MSN) degree from an accredited Nurse

Practitioner program.

 Current and unrestricted Nurse Practitioner license in GA

 Experience in hospice, palliative care, or home health settings is highly preferred.

 Valid driver's license and reliable transportation for patient home visits.
