What are the responsibilities and job description for the Case Worker position at A2D2?
We are seeking a Case Worker to join our team! You will provide emotional and administrative support to clients in either individual or group settings.
Responsibilities:
- Assess nature of clients' situations by interviewing and reviewing personal history
- Develop and execute individual treatment plans
- Determine appropriate milestones to gauge client progress
- Communicate client updates to all relevant parties
- Facilitate referrals to other healthcare professionals and programs
- Maintain accurate client documentation
Qualifications:
- Previous experience in social work, counseling, or other related fields
- Compassionate and caring demeanor
- Ability to build rapport with clients
- Strong leadership qualities
- Excellent written and verbal communication skills
Bachelor’s or Master's Degree in human services, a related field, or other degree deemed equivalent to those described, from an accredited college. A human services field includes social work, gerontology, psychology, psychiatric rehabilitation, special education, sociology, counseling, and human services counseling. Considerable experience working with emotionally troubled individuals and affected families, or equivalent experience as specified by VDBHDS/DMAS regulations. Must be able to show a minimum of one (1) year direct service to individuals and families. QPPMH meeting VDBHDS/DMAS regulations are also considered.
