What are the responsibilities and job description for the Community Liaison / Outreach Representative — Home Care position at A2 Home Care?
We're a non-medical home care agency seeking an experienced community liaison to grow our referral network in Washtenaw County
What you'll do:
- Build relationships with hospitals, skilled nursing facilities, senior living communities, geriatric care managers, elder law attorneys, and physical therapy clinics
- Conduct in-person visits and follow-ups with referral sources
- Represent our agency at community events and senior expos
- Track referral activity in our CRM
