All work sites are located on Staten Island
Full Time - 40 hours/week, Monday-Friday 7:00am-3:30pm
Job Requirements
- 3 years OPWDD experience
- AMAP Certification -in good standing for 2 years
- Valid Driver's License - Safe Driver Certification
- CPR/FA Certification
- Scip-R Certification
- Computer skills - Microsoft Office - EHR
- Understanding of HIPPA
Job Responsibilities:
· Checks medications daily to ensure adequate supply.
· Orders medications as needed.
· Review MAR for medication errors. Reports any medication errors to RN and RM.
· Ensures medication room has adequate supplies.
· MARS medications as assigned by the RN.
· Reviews health-related documentation for completeness- (i.e. bowel charts, fluid charts, sleep charts, menstrual charts,). Reports any omissions to RN and RM
· Prepares consults for each appointment.
· Maintains medical calendar and schedules appointments. Schedules transportation as needed.
· Attends medical appointments with individuals.
· Participates in tele-health appointments with individuals.
· Provides information to staff about appointments- (what is needed, what to tell clinician).
· Reviews consults after appointment and provides pertinent information to RN.
· Ensures that all necessary documents and prescriptions are obtained from physician.
· Visits hospitalized individuals to aid in communication and discharge planning.
· Updates electronic health record (i.e., health conditions, medications, healthcare providers, diet)
· Meets regularly with RN to discuss medical status and medical follow up needed.
· Meets weekly with RN and RM. Reviews upcoming appointments, tests, and outstanding tasks.
· Attends IDT meetings, semi-annual meetings and any additional meetings deemed necessary by RN.
· Any additional tasks related to the individual’s health care deemed necessary by RN.
Job Type: Full-time
Pay: $21.00 - $22.50 per hour
