Job Summary

Duties
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patients’ health status, including vital signs and overall condition, reporting any concerns promptly
- Support medication administration in accordance with medical directives and maintain accurate medical documentation
- Utilize Electronic Health Record (EHR) systems to document care activities, patient progress, and medical records securely and efficiently
- Provide specialized care for clients with dementia, Alzheimer’s disease, or other memory-related conditions, including memory care techniques
- Assist with catheter care, wound care, hospice support, and other medical procedures as trained and authorized
- Engage in light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment in compliance with HIPAA regulations
- Operate assistive devices like Hoyer lifts safely to prevent injury during transfers and mobility assistance
- Support clients in community settings or long-term care facilities such as assisted living or nursing homes when needed
Skills
- Strong patient observation skills to detect changes in health or behavior promptly
- Experience working with individuals with developmental disabilities or in senior care environments
- Knowledge of medical terminology and familiarity with EMR/EHR systems for accurate documentation
- Certification in CPR and first aid to ensure immediate response during emergencies
- Ability to perform catheter care, medication administration, and basic wound management confidently
- Compassionate caregiving skills tailored to dementia and Alzheimer’s care needs
- Excellent communication skills to collaborate effectively with clients’ families and healthcare teams
- Physical stamina for assisting with mobility tasks like using Hoyer lifts or helping patients move safely
- Respect for patient privacy and confidentiality in accordance with HIPAA standards
- Flexibility to work live-in shifts or part-time hours in home or community settings
