Position Title: Support Coordinator
Positions Available: one — One Part-Time (with potential to expand to full-time)
Schedule: Must be available during daytime hours
Location: South Jersey Regions
Reports To: Support Coordinator
Experience:
- Minimum of one (1) year full-time experience working with individuals with developmental and/or intellectual disabilities. Experience as a support coordinator for the NJ Division of Developmental Disabilities a plus.
Position Overview
A Simple Leap Support Services is seeking compassionate, organized, and person-centered Support Coordinators to join our growing South Jersey team. We are currently hiring one part-time Support Coordinator with the potential to transition to full-time based on caseload growth and performance.
In this role, you will help individuals enrolled with the New Jersey Division of Developmental Disabilities (DDD) navigate available supports, develop meaningful goals, coordinate resources, and participate in their communities in ways that reflect their preferences and strengths. This position requires a strong understanding of DDD guidelines, person-centered planning practices, and the Supports and Community Care Programs.
Responsibilities
- Conduct monthly contacts and quarterly in-person visits in accordance with DDD requirements.
- Develop, monitor, and update Individualized Service Plans (ISP) in iRecord while ensuring accuracy and compliance.
- Facilitate Person-Centered Planning meetings that empower individuals to identify goals, strengths, and preferences.
- Coordinate needed services including Community-Based Supports, Supported Employment, transportation, clinical supports, residential supports, and other resources.
- Maintain consistent and professional communication with individuals, families, guardians, and providers.
- Document all communications, service notes, and plan updates in iRecord in a timely manner.
- Identify and report safety concerns, abuse, neglect, or exploitation following mandated reporting requirements.
- Participate in ongoing training, supervision, and agency meetings to maintain compliance and remain informed of program updates.
- Adhere to HIPAA, Medicaid, and agency confidentiality standards.
Qualifications
- Bachelor’s degree
- Minimum of one (1) year full-time experience working as a Support Coordinator for the NJ Division of Developmental Disabilities (required).
- Experience with person-centered planning, iRecord documentation, and waiver services.
- Strong written and verbal communication, organizational skills, and time management abilities.
- Comfortable working independently while managing timelines and caseload responsibilities.
- Reliable transportation and willingness to travel across Camden, Gloucester, Burlington, Ocean, and Cumberland Counties.
Preferred Skills
- Knowledge of community resources and Medicaid-based services within New Jersey.
- Experience with NJCAT scoring, tier assignments, and DDD service navigation.
- Professional familiarity with crisis response, advocacy, and disability support systems.
Additional Requirements
- Valid driver’s license and acceptable driving record.
- Successful completion of state-required background checks.
Schedule & Growth Opportunity

Job Type: Part-time
Pay: $1,500.00 - $3,500.00 per month
