Each Case Manager will be assigned a “home program” for primary placement; however, the expectation is that all Case Managers operate inter-programmatically to provide unified, unsiloed, and comprehensive care. • Build therapeutic, professional relationships with clients using trauma-informed, strengths-based practices.

Case Management & Coordination

  • Provide regular one-on-one case management sessions, adjusting service plans as needs evolve.
  • Collaborate across programs to ensure continuity of care and equitable access to services, regardless of a client’s assigned housing program.
  • Facilitate connections to in-house resources (employment, education, clinical services, recovery supports) and community partners.
  • For NPTS clients, coordinate with the Office of the Chief Judge and electronic monitoring officials to ensure compliance and support successful re-entry.

Documentation & Compliance

  • Maintain timely, accurate case notes, service plans, and progress updates in accordance with program, funder, and regulatory standards.
  • Track and report client outcomes across programs, ensuring consistency in data entry and compliance.
  • Adhere to confidentiality and ethical standards of practice.

Team Collaboration & Inter-Program Work

  • Participate in interdisciplinary case reviews to promote inter-programmatic planning and accountability.
