Job Summary:
The Part-Time On-Call LVN provides skilled nursing care and support to hospice patients during after-hours and weekends.
Responsibilities:
- Respond promptly to on-call assignments for hospice patients during evenings, weekends, and holidays.
- Assess patient needs and provide appropriate nursing interventions in accordance with the plan of care.
- Communicate effectively with patients, families, caregivers, and interdisciplinary team members.
- Document all nursing care provided accurately and in a timely manner.
- Collaborate with the RN Case Manager and Hospice Medical Director as needed.
- Provide emotional support and education to families and caregivers.
- Maintain compliance with all federal, state, and local regulations as well as company policies and procedures.
Qualifications:
- Current LVN license
- Minimum of 1 year of nursing experience; hospice or palliative care experience preferred.
- Ability to work independently and respond calmly under pressure.
- Strong communication and critical thinking skills.
- Compassionate, dependable, and dedicated to quality patient care.
