Job Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality patient care in the comfort of patients' homes, ensuring their health and well-being. This role requires a strong background in nursing with a focus on case management, discharge planning, and patient education. Duties
- Conduct thorough assessments of patients' health status and needs in a home setting.
- Develop and implement individualized care plans based on patient evaluations.
- Manage case coordination, including communication with physicians, families, and other healthcare providers.
- Provide education and support to patients and their families regarding treatment plans and health management.
- Monitor patients' progress and adjust care plans as necessary to ensure optimal outcomes.
- Document all patient interactions accurately in compliance with HIPAA regulations.
- Assist with discharge planning for patients transitioning from inpatient to outpatient care.
- Provide specialized care for pediatric patients as needed, including behavioral health support.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- 1 Year of proven experience in home health nursing care.
- Strong knowledge of case management principles and utilization management practices.
- OASIS Experience
- Excellent communication skills and the ability to work effectively with diverse populations.
- Understanding of HIPAA regulations to ensure patient confidentiality and privacy.
- Ability to work independently while managing multiple priorities effectively.
