Overview
We are seeking compassionate and dedicated Home Health Aides (HHA) and Caregivers to provide exceptional in-home and community-based care to clients of all ages. This role involves supporting individuals with diverse needs, including seniors, those with developmental disabilities, and individuals requiring specialized care such as Alzheimer's, dementia, or hospice. The ideal candidate will have a strong commitment to patient well-being, excellent communication skills, and experience working in assisted living or long-term care environments. Duties
- Provide personal care including assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs and reporting changes to healthcare professionals
- Assist with medication management and administration following prescribed protocols
- Support patients with memory care needs, including Alzheimer's and dementia care strategies
- Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively
- Document patient information accurately using EMR/EHR systems and maintain detailed medical records in compliance with HIPAA regulations
- Prepare nutritious meals and assist with feeding as needed, ensuring dietary restrictions are followed
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment
- Support patients during hospice care or end-of-life situations with compassion and respect
- Assist with transportation to appointments or community activities when required
- Collaborate with healthcare teams to ensure comprehensive patient care plans are followed
Requirements
- Proven experience in home health, senior care, assisted living, or long-term care settings; experience in nursing homes or working with people with disabilities is preferred
- Knowledge of medical terminology, medication administration, and patient documentation standards
- Certification in CPR is required; additional certifications such as hospice care or dementia training are a plus
- Ability to work independently while maintaining excellent communication within a team environment
- Strong observational skills for patient safety and condition monitoring
- Experience working with EMR/EHR systems for accurate record keeping
- Compassionate attitude with a genuine desire to improve clients’ quality of life
- Valid driver’s license preferred for transportation duties; reliable transportation is necessary for home visits
- Ability to perform physical tasks such as lifting or assisting patients using Hoyer lifts or other equipment
- Flexibility to work various shifts based on client needs; some positions may involve live-in arrangements
