HOSPICE LICENSED PRACTICAL NURSE (LPN)

What You'll Do as a Hospice LPN
As a Hospice LPN, you provide direct patient care under the supervision of the RN Case Manager and play a vital role in supporting patients and families. Your responsibilities include:
- Delivering care according to the individualized plan of care
- Assessing patient needs and reporting changes in condition
- Identifying and reporting any signs of abuse
- Making recommendations for updates to the plan of care
- Counseling patients and families on advance directives such as living wills and healthcare surrogate forms
- Maintaining accurate, up‑to‑date patient records
- Building positive relationships with vendors and community partners
- Participating in Total Quality Improvement initiatives and training programs
- Assisting with new nurse orientation and discharge planning
- Participating in the on‑call rotation as needed
Your compassion and clinical skill help patients and families navigate end‑of‑life care with dignity and support.
Qualifications
- Active Oklahoma LPN license
- At least 1 year of clinical experience
- Ability to care for patients and families facing terminal illness
- Valid Oklahoma driver's license, reliable transportation, and vehicle insurance
- Acute care experience preferred but not required
- Strong interpersonal and communication skills
- Ability to work effectively as part of a team
Work Schedule
- Flexible hours between Monday and Friday, 8:00 AM - 5:00 PM
