Responsibilities:
- Conduct comprehensive in-home patient evaluations to assess functional abilities and develop individualized treatment plans
- Provide hands-on physical therapy treatments including therapeutic exercises, manual therapy, and mobility training
- Monitor and document patient progress, adjusting care plans as needed to meet evolving goals
- Educate patients and family members on home exercise programs, fall prevention, and safe mobility techniques
- Collaborate with interdisciplinary care teams including physicians, nurses, and occupational therapists
- Maintain accurate and timely clinical documentation in compliance with Medicare and state regulations
- Coordinate discharge planning and recommend appropriate follow-up care or community resources
- Active Physical Therapist (PT) license in the state of Indiana
- Graduate degree (DPT, MPT, or equivalent) from an accredited physical therapy program
- Home health or outpatient clinical experience preferred
- Strong knowledge of Medicare home health guidelines and documentation standards
- Valid driver's license and reliable transportation for home visits throughout Indianapolis
- Excellent interpersonal and communication skills with a patient-first mindset
- Ability to work independently and manage a flexible caseload
