A Love for Life assisted Living is a resident‑centered community in Albuquerque, NM, dedicated to providing compassionate support and high‑quality care for seniors. We are seeking an experienced, detail‑oriented Resident Care Coordinator to join our team.
Position Summary
The Resident Care Coordinator oversees resident care, ensures service plans are followed, supports caregiving staff, and helps maintain compliance with state regulations. This role is essential in ensuring residents receive safe, respectful, and person‑centered care.
Key Responsibilities
- Oversee and update individualized service plans.
- Perform routine resident assessments and observe changes in condition.
- Ensure care delivery meets state regulations and company standards.
- Coordinate communication with families, caregivers, healthcare providers, and hospice teams.
- Support and guide caregiving staff, including assisting with scheduling and training.
- Maintain accurate documentation, incident reports, and compliance records.
- Participate in resident care meetings and address care‑related concerns.
Required:
- High school diploma or equivalent.
- Minimum 2 years of experience in assisted living, senior care, long‑term care, or a related field.
- Strong communication, leadership, and organizational skills.
- Knowledge of state regulations for Assisted Living in New Mexico.
Preferred:
- CNA, Medical Assistant, or related certification.
- Experience in resident services, care coordination, or supervisory roles.
- Familiarity with electronic care documentation systems.
