Job Description
- Provides evidence-based nursing care in accordance with the interprofessional plan and ANA standards.
- Collaborates with healthcare team to ensure continuity and coordination of patient care.
- Acts as a patient and family advocate, performing point of care testing as needed.
- Assesses patient needs, develops, and implements individualized care plans.
- Evaluates educational needs and employs appropriate teaching strategies.
- Initiates discharge planning and facilitates smooth transition to next service or home.
- Ensures accurate and timely documentation of patient care activities.
- Shift: Nights, 7p-7:30a, 12-hour shifts, every other weekend rotation.
- Remote/on-site/hybrid: On-site at Blodgett with potential to float to BW - 4G Digestive Disease.
- Job duration: 12 weeks.
