RN Case Manager Hours

• Work schedule: Monday to Friday, 8am-5pm
• Can live anywhere on the east coast in EST time, but MUST be licensed in New York.

RN Case Manager Compensation

• The pay for this position is $38.46 hour paid weekly
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

RN Case Manager Responsibilities

• Engage telephonically with members and coordinate their care.
• Develop individualized care plans based on clinical information and member needs.
• Provide evidence-based disease management education to assist members in achieving health goals.
• Ensure interdisciplinary care team involvement in member care.
• Document care management activities meticulously in electronic health records.
• Connect members with health plan benefits and community resources.
• Support compliance with regulatory requirements and performance metrics.

RN Case Manager Requirements

• RN License in NY REQUIRED
• Can live anywhere on the east coast in EST time, but MUST be licensed in New York.
