RN Case Manager Hours
- Work schedule: Monday to Friday, 8am-5pm
- Can live anywhere on the east coast in EST time, but MUST be licensed in New York.
RN Case Manager Compensation
- The pay for this position is $38.46 hour paid weekly
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN Case Manager Responsibilities
- Engage telephonically with members and coordinate their care.
- Develop individualized care plans based on clinical information and member needs.
- Provide evidence-based disease management education to assist members in achieving health goals.
- Ensure interdisciplinary care team involvement in member care.
- Document care management activities meticulously in electronic health records.
- Connect members with health plan benefits and community resources.
- Support compliance with regulatory requirements and performance metrics.
RN Case Manager Requirements
- RN License in NY REQUIRED
- Can live anywhere on the east coast in EST time, but MUST be licensed in New York.
