HOME HEALTH LPN | DETAILS AND COMPENSATION:

• Location: Virginia Beach VA 23452 – 100% On-site
• Payrate: $37.30/hr
• Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
  • (on-call and overtime may be required occasionally)

HOME HEALTH LPN | SUMMARY AND HIGHLIGHTS:

The Home Health LPN role involves providing high-quality patient care, supporting treatment plans, and ensuring compliance with medical and safety standards.

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

HOME HEALTH LPN | RESPONSIBILITIES:

• Provide care in alignment with the patient’s individualized plan of care, while maintaining dignity and independence.
• Monitor vital signs and promptly report abnormal findings.
• Administer medications, feedings, oxygen, and ostomy care as directed.
• Support therapy routines established by Physical, Occupational, and Speech Therapists.
• Maintain infection control protocols and report potential communicable diseases or infections.
• Document care provided, progress toward goals, and patient outcomes accurately and thoroughly.
• Order necessary supplies, medications, and feedings to maintain adequate stock.
• Respond to patient incidents, complete incident reports, and ensure appropriate follow-up.
• Ensure medical equipment is functioning properly and report any issues for repair.

HOME HEALTH LPN | REQUIREMENTS:

• Education: High School Diploma or GED (Verifiable).
• Attendance: Commitment to mandatory attendance during the first 90 days.
• Active Licensed Practical Nurse (LPN) license (required).
• Wound care and IV experience (required).
• HCHB (HomeCare HomeBase) experience (required).
• Strong bedside manner and communication skills.
