Job Title: LPN – Continuing Care (Nights)Pay: $35–$42/hour, paid weeklyLocation: Grand Rapids, MI 49503Shift: Nights, 7:00pm-7:30am (12-hour shifts)Weekend Rotation: Every other weekendOn Call: NoneScrub Color: Navy BlueJob Type: 13-week to start W2 contract assignment with potential for extension or conversion to direct based on attendance, performance, and business needs.Job SummaryWe are seeking a dedicated Licensed Practical Nurse (LPN) to join a continuing care team focused on delivering exceptional patient care. In this role, you will work under the direction of an RN, mid-level provider, or physician to assess, plan, implement, and evaluate patient care. You will play a vital role in supporting patients and their families, ensuring compassionate, high-quality care in a long-term and rehabilitative setting.ResponsibilitiesCollect and monitor assessment data; report patient needs across physiological, psychological, sociocultural, and spiritual domains.Provide direct patient care as delegated by an RN or provider, following established policies and routines.Educate patients and families on treatments, medications, equipment, and care activities.Accurately document all care provided in electronic and/or written records.Communicate with patients and families in a respectful, compassionate manner.Collaborate with the healthcare team to ensure coordinated and continuous care.Maintain a clean and safe patient environment; report equipment issues promptly.Document all chargeable items per standard guidelines.Participate in quality monitoring activities as directed.Demonstrate ethical and professional behavior, respecting privacy and confidentiality at all times.Adhere to all safety initiatives and complete required competencies.Perform additional duties as assigned.QualificationsHigh school diploma or equivalentCompletion of an approved nursing programRequired: 2 years of LPN Experience.
