This will be full time.If you are interested in Hybrid Case Manager – Dual Eligibility Specialist Opportunity, please contact Michelle at 586-422-1171 or Mmansoor@alinestaffing.com.Hybrid Case Manager – Dual Eligibility Specialist HoursMust be able to work 8am to 5pm CST time zone M-FNonclinical, must live in the state of IL; must reside in any of the following counties (Cook, Winnebago, Lasalle, Winnebago, Rock Island)Will be able to work from home 1-2 days a week/ be in the field completing member visits Hybrid Case Manager – Dual Eligibility Specialist CompensationThe pay for this position is $28.34 an hour mileage reimbursementBenefits are available to full-time employeesA 401(k) with a company match is available for full-time employees.Hybrid Case Manager – Dual Eligibility Specialist ResponsibilitiesManage a caseload of 250–500 dual eligible members (Medicare/Medicaid) via telephonic case management.Assess members' healthcare and social needs and develop personalized care plans.Coordinate appointments, healthcare services, and connect members with community resources.Collaborate with healthcare providers to ensure timely and effective care.Help members navigate Medicare and Medicaid benefits.Advocate for members to ensure they receive proper care and services.Educate members on healthcare rights, benefits, and treatment options.Track progress and monitor care plans using care management tools.Address social determinants of health, such as housing and transportation, and connect members to resources.Participate in team meetings and share insights on complex cases.Follow up with members to ensure care goals are being met.Submit reports on case management outcomes and ensure compliance with regulations.Maintain cultural sensitivity and provide person-centered care.Hybrid Case Manager – Dual Eligibility Specialist RequirementsBachelor degree REQUIRED (psychology, social work, marriage and family therapy, counseling).Case management and discharge planning experience preferred2 years’ experience in behavioral health, social services or appropriate related field equivalent to program focusManaged Care experience preferredIf you think this Hybrid Case Manager – Dual Eligibility Specialist Opening is a good fit for you, please reach out to me - feel free to call, e-mail, or apply to this posting!
