Physical Therapist ResponsibilitiesPerform physical therapy evaluation and treatment of patients based upon physician orders/referrals.Complete comprehensive evaluations using appropriate assessment tools and procedures.Review patient records for current diagnosis, past medical history, precautions, and contraindications.Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence/function.Regularly reassess effectiveness of treatment plans and modify goals as necessary.Communicate patient needs and progress to the treatment team, physician, patient, and family members as applicable.Consult with medical providers regarding patient evaluation, treatment, progress, and discharge.Recommend and assist in securing special adaptive equipment and incorporate use in rehab program.Maintain and submit evaluation, treatment, and billing documentation per company and regulatory requirements.Adhere to company policies, HIPAA, corporate compliance programs, professional standards, and applicable laws.Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care.Attend, participate in, and/or conduct internal staff development programs and meetings; obtain continuing education as required.Physical Therapist RequirementsHigh School Diploma or GEDWV Physical Therapist LicenseAttendance is mandatory for the first 90 daysPhysical Therapist Preferred QualificationsHome Health experience with OASISHCHB experienceIf you think this Physical Therapist position is a good fit for you, please reach out to me – feel free to e‐mail or apply to this posting!
