About The Job
A Hug Away Healthcare is seeking a dedicated and compassionate Nurse Practitioner with wound care experience to join our hospice team on a per-visit basis. The ideal candidate will have prior experience working with a wound care company and be responsible for conducting face-to-face visits with our hospice patients, providing high-quality palliative and wound care, and ensuring the comfort and dignity of our patients during their end-of-life journey.
Key Responsibilities
- Conduct face-to-face visits with hospice patients in their homes or care facilities.
- Assess and manage patients’ symptoms, pain, wounds, and overall health status.
- Develop and implement individualized care plans in collaboration with the hospice team.
- Provide specialized wound care management to hospice patients.
- Educate patients, families, and caregivers on wound care and symptom management.
- Document patient visits and care provided in accordance with CMS guidelines and company policies.
- Respond to last-minute visit requests and provide timely care as needed.
- Open to caring for children and their families as well as adults.
- Current and unrestricted Nurse Practitioner license in Texas.
- Prior experience working with a wound care company is required.
- Minimum of 2 years of experience in hospice, palliative care, or wound care.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Comfortable with last-minute visit requests and flexible scheduling.
- Competitive per visit pay of $100 - $150 per visit.
