MINIMUM QUALIFICATIONS
Education: master's degree from an accredited college or university with a major in Behavioral Science, Social Work, Social Welfare, Marriage and Family therapy or Psychology.
EXPERIENCE:
A combination of minimum three years of case planning, case management, and fieldwork in a public or private agency as a case manager or closely related job title. Preferred ORR or FFA.
JOB DESCRIPTION
Under the Administrator's supervision, performs a full range of targeted and/or general case management that is strength based, needs-based, community-based, and family-driven. Assesses the needs of UACs in care, develops Individual Services Plans, screens for human trafficking concerns, facilitates the safe and timely release or discharge of children and youth, documents the prevision of services in case files. The case manager is responsible for developing and maintaining community relationships, networks, collaborations that improve the availability and quality of formal and informal services to families and children.
ESSENTIAL FUNCTIONS
1) Make case studies for the purpose of assessing problems and determining appropriate types and methods of treatment.
2) Investigate and provide services and referrals for clients where their physical or emotional welfare is involved.
3) Work with cases involving neglect, abuse, emotional or behavioral problems, physical/mental disabilities or other health conditions affecting a client's personality, family relationships, child or in-home care arrangements.
4) Develop intensive long- or short-term treatment plans to improve or restore individual or family functioning.
5) Conduct thorough psychosocial assessments on families.
6) Develop person-centered, strength-based case plans.
7) Assess family environment to determine potential or imminent threat to child permanency and stability.
8) Maintain all records up-to-date of case management and prepares reports.
