Duties
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance using proper patient transfer methods such as Hoyer lifts and manual handling techniques
- Monitor patients’ vital signs, observe changes in health status, and report findings accurately to healthcare professionals in compliance with HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation
- Support clients with medication administration following care plans and ensuring safety protocols are maintained
- Provide companionship and emotional support, engaging clients in social activities to promote mental well-being
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces
- Assist with transportation for medical appointments or community outings, ensuring safe patient transfer methods are used at all times
Requirements
- Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care in settings such as group homes, assisted living facilities, nursing homes, or home health environments
- Knowledge of dementia care, Alzheimer’s care, memory care, hospice care, and neurodevelopmental disorders such as autism
- Ability to perform patient monitoring including vital signs assessment and recognizing signs of medical emergencies; CPR and first aid certification preferred
- Familiarity with care plans, medication administration procedures, patient personal care assistance, and HIPAA compliance standards
- Strong communication skills for effective client interaction and documentation; experience working with diverse populations including geriatrics is a plus
- Physical ability to lift heavy objects safely (heavy lifting) and assist with manual handling tasks; experience operating patient transfer devices like Hoyer lifts is advantageous
- Valid driver’s license or reliable transportation for client outings; experience providing personal transportation assistance preferred
