We are seeking a dedicated and compassionate Care Manager to join our team. The ideal candidate will play a crucial role in providing support and guidance to individuals facing various challenges, including addiction, mental health issues, and family dynamics.
Position Summary:
The Care Manager will be responsible for developing personalized care plans, facilitating access to resources, and ensuring the well-being of clients through effective supervision and intervention strategies.
Primary Responsibilities:
- Develop and implement individualized care plans tailored to the unique needs of each client.
- Supervise and mentor staff members, fostering a collaborative environment that promotes professional growth.
- Utilize motivational interviewing techniques to engage clients in their treatment process.
- Provide addiction counseling and crisis intervention services as needed.
- Conduct assessments to identify client needs and coordinate appropriate services.
- Collaborate with community resources and stakeholders to enhance service delivery.
- Maintain accurate documentation of client progress and interventions in accordance with organizational policies.
- Support workforce development initiatives by training staff on best practices in social work and child welfare.
- Facilitate group sessions focused on crisis management and coping strategies for clients.
Experience, Qualifications, and Skills:
- Proven experience in supervising teams within a care management or social work setting is preferred.
- Strong background in motivational interviewing, addiction counseling, and crisis intervention techniques.
- Experience working with children, families, or vulnerable populations is highly desirable.
- Familiarity with child welfare systems and workforce development programs is a plus.
- Excellent communication skills with the ability to build rapport with clients from diverse backgrounds.
- Demonstrated ability to manage crises effectively while maintaining a calm demeanor under pressure.
