Responsibilities
- Conduct comprehensive assessments to identify client needs and develop individualized care plans.
- Utilize motivational interviewing techniques to engage clients and encourage positive behavior changes.
- Provide ongoing support and monitoring of client progress, adjusting care plans as necessary.
- Collaborate with multidisciplinary teams, including social workers, healthcare providers, and community resources.
- Facilitate access to services such as Section 8 housing assistance, mental health resources, and addiction counseling.
- Supervise and mentor junior staff or interns in best practices for client care and crisis management.
- Implement crisis intervention strategies to support clients in high-stress situations.
- Maintain accurate documentation of client interactions, progress notes, and service referrals.
Experience
- A background in social work or a related field is preferred.
- Experience with crisis management and intervention techniques is essential.
- Familiarity with Medicare processes and child welfare systems is advantageous.
- Skills in supervising and mentoring staff or volunteers are beneficial.
- Previous experience in addiction counseling or working with diverse populations is a plus.
- Strong communication skills with the ability to build rapport with clients from various backgrounds.
