- Conduct in-home client assessments
- Assist in developing personalized care plans
- Perform supervisory visits to ensure quality care
- Support caregivers and collaborate with families

Qualifications:
- Willing to travel (we will pay mileage)
- Active LPN license in Georgia
- Home care or case management experience preferred
- Strong communication & organizational skills
- Valid Driver’s License
- Social Security Card
- Recent negative TB test
- Current CPR certification
