We're seeking an experienced, compassionate Registered Nurse (RN) with a background in home care, home health, hospice, or community-based care to provide clinical oversight for our growing agency.
This is a PRN, 1099 Independent Contractor position offering flexible scheduling with opportunities for continued growth. You will manage your own visit schedule while meeting required compliance timeframes.
Service Area: Fulton, DeKalb, Cobb, Clayton, Cherokee, Gwinnett, Douglas, Rockdale, and Paulding Counties

Primary Responsibilities
Client Assessments & Supervisory Visits
- Complete new client nursing assessments (same-day or next-day turnaround when needed)
- Conduct required 60-day supervisory visits
- Perform annual reassessments
- Develop and update individualized client care plans
- Travel to client homes using your own reliable vehicle
Clinical Oversight & Documentation
- Review caregiver documentation and ADL records
- Ensure documentation complies with DCH, HFRD, Georgia Medicaid requirements, agency policies, and nursing standards
- Complete documentation accurately, timely, and professionally
- Identify changes in condition, safety concerns, or care needs and document appropriately
Escalation & Care Coordination
- Immediately notify agency leadership of falls, incidents, injuries, abuse concerns, safety risks, hospitalizations, or significant changes in condition
- Provide clinical recommendations when higher levels of care or additional services are needed (PT, OT, Skilled Nursing, Rehab, Nursing Facility, etc.)
- Participate in regular communication with agency leadership regarding client care
Weekly Documentation Review (Remote)
- Review and sign off on caregiver documentation inside our care management software
- Sign-off is completed weekly for the previous week
- Performed remotely
Our Expectations

Our RN partners are expected to communicate promptly, meet required documentation deadlines, and work collaboratively with our office team to ensure our clients receive safe, high-quality care. In return, you'll receive organized scheduling, administrative support, and leadership that is accessible and responsive.
Compensation
- Initial Nursing Assessment: $70 per visit
- Annual Reassessment: $70 per visit
- 60-Day Supervisory Visit: $60 per visit
- Weekly Care Note Sign-Off (Remote): $80 flat rate
- In-Office Work (Rare, If Needed): $27 per hour
Compensation is paid per completed visit and flat rate for documentation review. No hourly compensation is provided for field visits, and there is currently no minimum visit requirement.
Qualifications
- Active Georgia Registered Nurse (RN) License
- Minimum of one (1) year RN experience
- Home Care, Home Health, Hospice, or Community-Based Care experience strongly preferred
- Current CPR and First Aid Certification
- Current negative TB test
- Ability to pass a background check and drug screen
- Reliable transportation and willingness to travel throughout the service area using your own vehicle
- Laptop, tablet, or iPad with reliable Wi-Fi for documentation
- Personal phone and internet service
- Comfortable using electronic care management software
- Strong clinical judgment, documentation, communication, and organizational skills
- Ability to work independently while managing schedules and documentation deadlines
