Position Summary
As a PRN Field Registered Nurse (RN), you will deliver high-quality, skilled nursing care directly in the patient’s home. You will play a vital role in promoting recovery, safety, and independence while ensuring accurate and timely documentation that supports excellent clinical outcomes.
Key Responsibilities
- Provide skilled nursing care, including medication administration, wound care, IV therapy, and phlebotomy
- Complete OASIS documentation in accordance with Medicare guidelines
- Deliver holistic, patient-centered care following each individualized plan of care
- Educate patients and families to support safe and effective self-care at home
- Maintain availability for occasional emergency coverage of unexpected open visits
- Maintain exceptional organization, communication, and clinical judgment
Qualifications
- Location: Duval County and surrounding areas
- Active Florida RN License – clear and in good standing
- OASIS experience required
- 1 year of Home Health nursing experience required
- 2 years of nursing experience preferred
- Alzheimer’s & Dementia Training (or willingness to complete)
- Ability to pass a Level 2 Background Screening *
- Experience with Electronic Medical Records (Kinnser/WellSky preferred)
