Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or wheelchairs.
- Monitor and observe patients closely, noting changes in condition or behavior and reporting findings accurately in medical documentation or electronic health records (EHR/EHR systems).
- Perform basic tasks such as taking vital signs, catheter care, or assisting with medication administration following prescribed protocols.
- Support clients with meal preparation, feeding, laundry, cleaning, and maintaining a safe and hygienic home environment.
- Document all patient interactions thoroughly while adhering to HIPAA regulations to ensure confidentiality of medical records and personal information.
- Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care techniques and behavioral management strategies.
- Collaborate with healthcare teams using electronic medical record systems to ensure seamless communication about patient needs and progress.
Qualifications
- Proven experience in home health, senior care, assisted living, nursing homes, or long-term care environments.
- Knowledge of medical terminology coupled with CPR certification to handle emergency situations confidently.
- Experience working with individuals with developmental disabilities or disabilities requiring specialized attention such as hospice or dementia care.
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping.
- Ability to perform patient observation accurately and respond promptly to changing conditions.
- Strong skills in medication administration, vital signs measurement, catheter care, and use of assistive devices like Hoyer lifts.
- Excellent caregiving skills combined with compassion, patience, and respect for client dignity.
