A Blessing Personal Homecare Services is seeking a dedicated and experienced Field Supervisor / Case Manager to join our team and support our Home and Community-Based Services.
JOB DESCRIPTION
Supervisory Responsibilities
Supervisor shall be responsible for conducting an annual evaluation on DSW. The evaluation shall include reviewing individual cases, providing constructive feedback, and assisting staff to provide services in a more effective manner. Supervisor shall also conduct:
- Uphold the company’s vision and mission statement and provide the greatest quality in-home care to the clients and families that we service.
- Collaborating with leadership team, implementing strategic plans for the company’s development, and managing relationships with stakeholders while still striving for company growth.
- Quarterly unannounced visits to recipient’s residence to observe service delivery.
- Train employees who can carry out our company’s vision and mission statement.
- Monitor company operations and ensure employees and business practice comply with HCBS regulations.
- Review payroll.
- Representing the company at corporate events (marketing, job fairs, etc.) that strengthens our company and communication.
Hours of Supervision: 8:00 AM- 4:00 PM Monday-Friday in office and continuously available to DSW workers by telephone or answering services after hours.
Supervisors Qualifications
The supervisors must be at least 25 years of age or older at the time the offer of employment is made. Verification of age must be maintained in each employee’s personnel record.
Direct care supervision must be full-time employees and meet one of the following minimum education and experience qualifications:
A bachelor's degree in human services- related field: social work, psychology, sociology, physical therapy, or two (2) years of paid experience in human services-related field providing direct services to the elderly and/ or persons with disabilities; or
A high school diploma or GED from an accredited school and five years of paid experience providing direct care services to the elderly and/ or persons with disabilities.
Starting Salary: $34,000.00
