Under the general supervision of the Clinical Supervisor, the Community Support team member will provide comprehensive support services to clients with behavioral health issues and their families. The position includes client care in a primary care setting and in the community, with direct contact with people who require empathy and sensitivity. The Community Support team member will be responsible for establishing and maintaining interpersonal relationships with clients and their families. This individual will function in a multidisciplinary team providing care for clients with addiction, social, and psychological needs and will receive intensive, ongoing training in these areas. The Community Support team member must be passionate about delivering excellent client care, exercise good judgment and initiative, work independently, and demonstrate strong organizational and time management skills. This position offers the opportunity to exercise sound judgment and initiative, work independently, and grow professionally. Professional demeanor and appearance are required.
Roles and responsibilities will include the following, but not limited to:
- Participate in the intake process by collecting, organizing, and analyzing information about individuals or families.
- Provide direct casework services to individuals or families regarding social involvement, health needs, and community life.
- Prepare and collaborate on various reports and develop case presentations on assigned individuals or families.
- Perform regular visits to provide client and family support and education.
- Coordinate appropriate programming, resources, and services for social development and in preparation for individuals or families.
- Collect data and write brief reports to meet the program's evaluation needs.
- Complete IM CANS Recerts.
- Make appropriate referrals to health and social service agencies. Referrals may include application assistance and client advocacy within and external to the organization.
Qualifications:
- A high school diploma or equivalent.
- Experience with navigation of local medical and social support systems preferred.
- A minimum of two years’ direct experience.
- Experience with Microsoft Office applications or other similar software applications is required.
- A valid driver's license is required.
- Minimum age requirement: Must be at least 18 years of age
