POSITION DESCRIPTION

TITLE: Case Manager

PRIMARY FUNCTION: Responsible for providing guests with housing-focused case management to meet their short-term and long-term goals. This includes, but is not limited to, housing referrals, housing search and application assistance, community resource referral(s), advocacy, crisis intervention, and supportive counseling. Case Manager will have crisis counseling and housing assistance focus. 

ESSSENTIAL JOB RESPONSIBILITES: 

Guest Case Care: 

  • Meet with guests to assess / resolve emotional crises and / or for basic supportive counseling as needed, delivering services through a trauma-informed lens.  
  • Complete intakes with new guests and enter all required information into data management system. 
  • Ensure that all guests receiving services have an intake completed.
  • Proactively identify guests who need to meet with a Resource Advocate for light touch services, including but not limited to, identification assistance, benefit assistance, application assistance, appointment scheduling, transportation assistance and prescription assistance. 
  • Create case plans with guests individualized to their short-term and long-term goals.  
  • Secure, coordinate, and monitor the delivery of individualized services to meet the needs of guests, prioritizing housing services, emergency health, and mental health including obtaining Federal, State, and local benefits. 
  • Proactively identify guests who require follow-ups and complete case management sessions and routinely engage with regular guests to ensure needs are being met. 
  • Complete proper documentation of all services provided, including but not limited to, case management sessions, intakes, referral and advocacy efforts, financial assistance, appointments with visiting professionals / agencies, etc. 
  • Document all case management sessions and notes in data management system. 
  • Serve as liaison between guests and other community organizations, and as an advocate for guests as needed. 

Stewardship & Collaboration: 

  • Assure smooth flow of services by assisting visiting professionals / agencies as needed. 
  • Serve as point person with community-based referrals and/or partnerships. 

Communication: 

  • Interact with local emergency services as needed for guests in crisis. 
  • Utilize walkie-talkies daily to communicate with team members on site at the Day Resource Center, and ensure your walkie-talkie is at the charge station prior to the end of your shift.  
  • Answer phone(s) and provide information or transfer calls as needed. 
  • Check voice mail and return calls or delegate accordingly. 
  • Ensure the delivery of quality and appropriate services through participation in any team meetings, staff meetings, and ongoing training / professional development opportunities.  
  • Maintain appropriate boundaries with guests and demonstrate sound judgement.  
  • Document all crisis intervention / counseling in data management system and give a report to supervisor. 
  • Notify supervisor in case of any emergency / crisis and proceed as instructed.  
  • Report any abuse / neglect to CPS or APS and document/report to your supervisor. 

Leadership Development: 

  • Serve as point of contact for onsite visiting professionals and keep staff notified of their status. 
  • Assist Resource Advocates at front desk as needed. 
  • Independently manage workflow amidst multiple competing priorities. 
  • Participate in organizational programs and/or events as needed. 
  • Enforce safety and behavior guidelines. 
  • Attend professional development opportunities / trainings as requested / required. 
  • Assume other duties as assigned by supervisor. 

SKILLS/KNOWLEDGE REQUIRED: 

  • Four-year degree in related field from an accredited college or university preferred. 
  • Three to five years of applicable experience preferred. 
  • Strong communication skills, both oral and written. 
  • Proficiency in Microsoft Office (including Excel and Outlook), and ability to learn other programs as required. 
  • Ability to conduct one’s self in an assertive, professional manner, as well as exhibit professional boundaries. 
  • Ability to work well in a team environment.  
  • Ability to tolerate a challenging and fast paced environment. 
  • Demonstrate organizational, staff, and project management abilities, including triaging guest needs in a fast-paced environment. 
  • Working knowledge of addiction / mental illness and how it might impact guest behavior. 
  • Working knowledge of the issues surrounding homelessness and poverty. 
  • Knowledge of community resources.  
  • Valid Arkansas State Driver’s License. 

PHYSICAL REQUIREMENTS/WORK ENVIORNMENT: 

  • Must be able to sit, stand or walk for extended periods. 
  • Must be able to lift up to 50 pounds and carry up to 10 pounds for short periods of time. 
  • Must be able to participate in physical activities throughout the facility, on the property, and / or community.
