Department: 39109 Burlington Walworth - Hospice Status: Full time Benefits Eligible: Yes Hours Per Week: 40 Schedule Details/Additional Information: Monday through Friday 8:00am to 4:30pm. Position is currently hybrid - telework/in office. Local travel occasionally. Major Responsibilities: Maintains staff schedules. Schedules patients for admission and home health aide visits. Coordinate additional staffing resources (SCT, agency) when staffing assistance is needed. Coordinate Face to Face (F2F) visits for physicians/APNs. This requires detailed knowledge of the coverage area for each staffing resource/vendor and any restrictions and/or limitations for each resource/vendor. Responsible for support function and timeliness of execution to meet the required regulations. Monitor specified Epic reports and work ques per defined timelines. Responsible for preparation of weekly IDT meetings. Ensures professional courtesy for effective triage/routing of phone calls and patient privacy. Answers all inbound telephone, screens calls and takes messages. Take intake calls with intake coordinator is unavailable. Performs other duties as assigned. MINIMUM EDUCATION AND EXPERIENCE REQUIRED License/Registration/Certification: A valid driver’s license issued by the Division of Motor Vehicles.
