Job Overview
We are seeking a dedicated and experienced Lead Case Manager to join our team. This role blends direct client support with supervisory responsibilities, ensuring high-quality service delivery and staff development. - Must be actively working toward a clinical practice license (e.g., LCSW, MFT, ACSW, or AMFT).
- Minimum of 2 years providing direct services to at-risk populations.
Responsibilities
- Conduct client intakes, assessments, and coordinate treatment plans.
- Identify client needs through therapeutic and psycho-social evaluations.
- Oversee case management tasks and guide staff on best practices and documentation.
- Ensure compliance with TILP goals, discharge planning, and progress tracking.
- Lead treatment team meetings and support decision-making using client data.
- Supervise staff development through training, coaching, and performance reviews.
- Conduct audits and implement performance improvement initiatives.
- Manage departmental budget and monitor service efficiency.
- Maintain accurate records for both client services and staff oversight.
- Participate in training, compliance reviews, and administrative meetings.
- Advocate for client needs while upholding policy and quality standards.
