What are the responsibilities and job description for the Shared Living Provider position at 3Rivers, Inc?
Shared Living Providers are contracted through 3Rivers to support an individual with intellectual disabilities or autism according to their person-centered plan. These individuals live in the home with the Shared Living Provider.
The Shared Living Provider works with the individual on all aspects of daily living, which may include administering medications, transportation, coordinating with other care providers, support medical appointments, improving safety skills, interpersonal skills development, nutritional support, behavioral interventions, and personal care. Shared Living Providers are members of an individuals multi-disciplinary team.
